JSO Releases Video, Details in West 22nd Street Officer-Involved Shooting

The Jacksonville Sheriff’s Office has released preliminary details and video related to an officer-involved shooting that occurred on West 22nd Street on January 11, 2026.

The incident resulted in the injury of Samuel Cook, 35, a convicted felon who is prohibited from possessing firearms. Based on his actions before and during the encounter, Cook has been arrested and charged with Attempted Murder of a Law Enforcement Officer, Aggravated Assault on a Law Enforcement Officer, and Possession of a Firearm by a Convicted Felon.

According to investigators, at approximately 6:40 p.m., District 1 patrol officers and members of JSO’s Citywide Task Force responded to a residence on West 22nd Street following a complaint that individuals at the home were dealing drugs and that at least one person was armed. Officers Mills and Lyons positioned themselves near the residence in an unmarked, covert vehicle.

At around 7:15 p.m., Cook approached the officers’ vehicle from behind, walked to the front, and pointed a handgun at the officers while they were seated inside. Both officers fired their JSO-issued handguns at Cook. Cook then fled eastbound on West 22nd Street, prompting the officers to exit their vehicle and pursue him on foot.

During the chase, Cook fired multiple shots at the officers. He eventually dropped his handgun on West 23rd Street, where officers were able to take him into custody. Officers immediately rendered medical aid until paramedics arrived. Cook was transported to the hospital and was reported to be in stable condition at the time the critical incident briefing was recorded.

A firearms expert later examined the handgun used by Cook and determined it had been connected to a previous shooting that occurred in November 2025.

As with all officer-involved shootings, the State Attorney’s Office will conduct an independent review to determine the legality of the officers’ actions. JSO will also complete an internal review to ensure departmental policies were followed. Both reviews remain pending.
